Thank you for choosing to give the Gift of Life. Signing and carrying a donor card is an important step to becoming a donor. In North Carolina, a properly executed (signed, witnessed & dated) donor card is now a legal document for organ/eye/tissue recovery.

While it is very important to carry the card and also a good idea indicate your desire to donate on your driver's license, the most important step is to tell your family that you want to be a donor. In North Carolina, if you do not have a signed donor card, your legal next-of-kin (usually an immediate family member) will be asked to give permission for organ, tissue, and eye donation.

Without knowing your wishes, your family may say no to donation. Since the donor card requires the signatures of two witnesses, sign the card in the presence of your family and have them be your witnesses. Then, they will know you want to be a donor.
